The Southeast District Health Department (SEDHD) invites the residents of Johnson, Nemaha, Otoe, Pawnee, and Richardson Counties to participate in a Community Health Assessment.  
Every three years, SEDHD partners with local hospitals to complete a Community Health Assessment.  
The goal of this assessment is to identify strengths and areas of improvement regarding the health of the community and incorporates data collected from organizations including the Nebraska Department of Health and Human Services, County Health Rankings, and the Centers for Disease Control and Prevention to compare our communities to those across the state and nation.  
This data provides SEDHD and local hospitals with a foundation for developing and implementing a Community Health Improvement Plan which identifies priorities in improving the health and quality of life for Southeast Nebraska residents.
As a part of the Community Health Assessment, SEDHD is currently conducting a community-based survey.
The purpose of this survey is to identify how residents perceive the quality of life in their community, the most important health issues, and the behaviors that have the greatest impact on the health of their community.  
Through this feedback, SEDHD and area healthcare partners will be able to develop actionable steps to improve the health of Southeast Nebraska.
